Office Policy

On your initial visit, please bring with you any requested items (such as X-rays, CT or MRI scans, etc.) as well as information concerning insurance coverage, workman's compensation or Medicare coverage. Co-payments are due at the time of your visit.

Occasionally the appointment schedule may be disrupted by a seriously ill patient who needs to be seen immediately. Your understanding in these situations is greatly appreciated.

Requests for prescription renewals are processed during normal office hours 8:00 a.m. to 5:00 p.m., Monday through Friday. Please allow 24 hours for your request.

Please see information below for procedures if you have an emergency after normal office hours.

If you have any questions or problems during normal office hours please call our surgical coordinator or patient coordinator. If your questions cannot be answered, your doctor will be contacted and respond directly to you.
